Introduction

The lack of integration can result in miscommunication among staffs and higher authorities as well create discord in the systematic procedural of an organisation. In reference to this report, here will be an explanation of the usage of System thinking and Archetypes in designing a State Value Stream Map reducing the patient turnaround time. The public hospital of Southampton named as Southampton General Hospital will be the base for the procedure where the activity will be taken place.

The patient turnover time (TAT) is the delivery of reports or specimen of particular patients in time, so that further activities can be carried out successfully. There are delays which can hamper the systemic procedure of the hospital as well delay the right treatment of the patients on time. The State value Steam map will help in resolving the issue so that there is overall development in the hospital and can get maximum patients to get their treatment in right time.

Identification and Analysis of the System Archetypes that may impede performance 

 System archetypes are used in various purposes and understanding long term purposes needed to solve the issues in an organisation. They are generally built up for analysing behavioural patterns. There are many system archetypes that are used in synchronising the activities of the organisation. Archetypes are the tool for analysing the behavioural patterns which helps in converting the dynamics of the organisation. These are dependent on heuristic which are the analysing tools for the better understanding of the mental dynamics or shortcuts. The Value Stream mapping helps in dividing the work into loops, where it has been observed that a particular event is depended o the next set of activities. For example, the hospital staffs look after to the daily development of the patients; take their blood tests, samples to the laboratory to get the desires results. Depending on these results the doctors can proceed with further treatment of the patients. The usage of the Value Stream process will help in getting the correct performance and will also help in understanding the flaws that must be eradicated to reduce the employee turnover time.

In the present State value Stream model there has been the availability of prolonged processes where the patients are checked for their viral statistics, medical history before examining. The patients are first examined by the nurses and them the check up report are send to the laboratory for further analysis. This will help in building up the records which will help the doctor to check the patients. The patient turnaround Time is thus prolonged as it waits for the report of the patients which can deteriorate the health of the patients, the customer can be dissatisfied with the process of treatment and this will eventually be dissatisfying for the patients. The patient turnaround time has been a topic of discussion in the emergency department of the hospital’s or care providing organisations. The inclusion of triage to providing shock rooms along with well equipped rooms for the patients involve the reduction of patient turnaround time. Including various equipments with technological support can improve the processes of the hospitality organisations and can gain competitive advantage in providing service to the customers.

Recommended New System Value Stream Map with desired reduction in patient turnaround time

In the recent technological era the development and utilisation of technological advances are been applied to gain reports within limited period of time. The generation of report very 12 to 24 hours will help reduce the patient turnover time and help in serving their customer in more numbers and in a much fecund manner. 

The utilisation of value stream Model will help in lean manufacturing of product, in case of this hospital it the services rendered to the customers and reducing the patient turnover time. The state value Stream model gives a detailed process represented in a procedural order the steps in which the work will be commenced and it will be documented as per the reduction of turnover time. The lean manufacturing Model represented by the help of Value Steam Model helps in waste management, reduction in processing time, and the overall improvement of the process cycle in rendering quality services to the customers. Therefore, it has been understood that Value Stream model has been used to get lean manufacturing improvements (Bharaj & Bhalla, (2016).

The advanced value stream model will be represented by the help of new technological advances, like the ADE process which helps in advanced drug examination. The parent is examined and the effect of certain drugs and tests will be extracted within 12 to 24 hours eventually reducing the patients wait time and overall patient turnaround reduction. In this advanced model, where the State Value stream model has taken the support of the ADE system which helps in analysing the quick reports for the patients and this helps in the doctor and nurse giving quick services to the patients (Nee et al.,(2013)). The rendering of report within time, will help the organisation to handle more customers at a time and also deliver quality services which will help the patients get accurate results for their treatment and desired attention is given for the patient to better in a much faster process.

In this process, the patient will be first observed by the nurse, pharmacist or the doctor it; there will no prolonged processes involved where the nurse and the doctor separately check the patients. The patient is treated thoroughly and sends to the examination laboratory for further tests, the reports will be rendered within 24 hours which will make the process more simplifies and faster in proves and the treatment of the patient is not in waiting list. The ADE system is also used in advanced medical treatment, where the effect of drug is analysed on the patient, and further replacement or betterment of treatment is decided by the doctor. The application of technological services helps in reducing the cost and time of the services and eventually lowering the patient turnaround time

Discussion on the intended and unintended consequences of the modified system

The new model can bring many positive as well as negative consequences which can be controlled by taking positive measures. The old model consist of multiple steps which through prolonged ensured quality treatment, but the involvement of advanced technology have made the work of the hospitals much easier and therefore they have to handle more customers which is eventually profiting both the parties. The involvement of advances technology have helped the hospitals and the doctors to handle more than one patients and the patient are getting reporting a shorter time which is helping them, in getting faster and accurate treatments (Verrier & Caillaud, (2016).

There are many clinical guidelines that can be given to the patients which have their own intended and unintended consequences, as they can cause disparities (Ruohoaho et al., 2018). The patients suffering from cancer or diabetes, or more serious chronic diseases gave to go through series of process for their treatment where normal people don’t have to go. In this case, the problem of patient turnaround increases when after the involvement of advanced technological equipments. Therefore, in the long run the hospitals and care providing organisations must propose more clinical actions than the usual practice guidelines.

It has been observed that the regular practice guidelines used in the hospitals does not reduce patient turnaround time and there is possibility of unintended consequences. Nevertheless, the involvement of new practice guidelines is necessary to reduce unintended consequences and patient turnaround time.

Conclusion

The following study has depicted the processes that must be involved in getting limited patient turnaround time. The system archetypes have been initially explained to cater to the application of the Value stream Model to synchronise the activities of the hospital. The first model was a prolonged process which increase the waiting time of the patients and then the revises model has been explained by which it has been represented that the use of advanced technological services and the use of ADE(Advanced Drug examination) in reducing the time of the patient turnaround time. 

Recommendation

It has been observed that United Kingdom has a lot of variations in the practice guidelines to adapt to the situations and treatments of patients. this can be applied to decrease the patient turnaround time

The use of more archetypes can also secure the procedural activities of hospitals and save time and money 

The physician and the nurses should have good decision making skills while handling emergencies, as patients faces many consequences of increment in their diseases due to the lack of decision making skills in the time of emergencies

Eradicating equalities in treating patients can develop the hospital in a sustainable process and develops in the socio- economic status.

Medical records should be kept intact to reduce the time of patient turnaround time

There must be availability of quality assurance experts and the process of auditing also helps in gaining productivity and profitability for the hospital.

There should be documentation of every patient and its ongoing procedures of treatments

Improvising the quality care can also increase the healthy practice of the hospital, the accurate standards in medical care will help the company gain more profitability and it will eradicate the unintended consequences and malpractice of the health organisations.
